Updated Provider Agreement Information

All current Ohio Ryan White Part B Program Providers and all new Ohio Ryan White Part B Program Providers must complete an updated Provider Agreement.

Current Ohio Ryan White Part B Program Provider Agreements expire on March 31, 2010.

In addition to the Provider Agreement, current and new Providers must also complete the Affirmative Action Program Verification Form.  For instructions on submitting the Affirmative Action Program Verification Form, click here

Community Planning

The Ohio AIDS Coalition collaborates with the Ohio Department of Health to ensure consumer driven community planning for Ohio’s Ryan White Part B Program. This is accomplished through the establishment of Consortia planning bodies throughout the state.

Funded by Part B of the Ryan White Treatment Modernization Act, Consortia exist to support and facilitate the provision of coordinated, comprehensive health and support services to people infected and affected by HIV/AIDS.

The Consortia exist for the purposes of prioritizing and preventing duplication of services at the local level.  Through the use of existing community-based services networks, the city/county consortia model ensures HIV infected individuals within the state receive the most efficient access to health care and support services and encourages cooperation among selected city/county agencies.

Goals of Consortia

Activities Undertaken by Consortia Across Ohio:

Each consortium across Ohio has a designated time and place for meetings which are generally held monthly. Each consortium also holds different events, trainings, and forums for community members, consumers, and those who work to provide services to individuals living with HIV/AIDS.
